Provides nursing care to assigned patients utilizing the Nursing Process to assess, plan, implement, and evaluate patient service based upon a physician's plan of treatment. Coordinates care delivered by other health disciplines for assigned patients.    

RESPONSIBILITIES:                                               

A. Assessment of patient’s need 

B. Development of care plan in collaboration with other health disciplines       

C. Implementation and coordination of plan of treatment      

D. Evaluation of patient’s progress toward achievement of goals and planned outcomes  

E. Professional Conduct

QUALIFICATIONS:      

1. Registered Professional Nurse licensed in State of Illinois.

2. Minimum of one year nursing experience in a hospital or home health agency.           

3. Demonstrates good assessment skills with ability to work with minimal supervision.

4. Has a current valid Illinois Driver's License and current required automobile insurance, if driving on agency business.

5. Proof of ability to perform the essential job functions and is in good health as indicated in a current physician's statement of health status.

6. Must not pose "direct threat" to the health or safety of the individual or others in the workplace or in the performance of job.
