Program Consultation
• Initiate, build and maintain relationships with downstate DCFS monitors.
• Attend downstate provider meetings e.g. foster care, TLP, and ILO.
• Educate agencies on DCFS Policies and TPSN Procedures that relate to assessment, service provision, documentation, reporting, investigations, etc for pregnant and parenting clients.
• Facilitate quarterly meetings with program staff and DCFS monitors to answer questions, provide guidance, attend case consultations and audit records.
• Facilitate communication between case work agencies and PPT Specialty Providers.
• Assess program needs as it relates to resources and training in working with TPSN clients in specific regions and develop a plan to assist and support which may include training.
• Provide TPSN in-service trainings for PPT agencies as it relates to working with pregnant and parenting clients as needed e.g. staff turnover, deficits in service delivery.
• Build relationships with external agencies (resources) and provide referrals for services when applicable.

Case Consultation
• Educate service team members on client rights according to the Hill Consent Decree.
• Advocate for client needs and rights when attending client staffings and court hearings.
• Facilitate and participate in staffings, (Clinical, CAYITs, Discharge, Placement, Hospitalization and Emergency) with pregnant/parenting clients, which includes the child and family team (GALs, therapist, foster /bio-family, when appropriate) to assess service needs and implementation of recommended services.
• Provides clinical expertise to staff when the individual teen or their placement is unstable or in a crisis situation.
• Provides ongoing consultation regarding effective and strategic crisis intervention with clients.
• Demonstrates insight regarding client’s strengths and needs and incorporates this when planning services and interventions and when making recommendations.

Case Conceptualization
• Makes plans and decisions based on clinically and developmentally appropriate, best practice, and ethical considerations.
• Uses population specific knowledge and expertise in developing case plans and intervention strategies.
• Understands the Mental Health diagnoses of clients and considers those needs in planning services.
• Considers any cultural factors that may be impacting the client or the appropriateness of services, and proficiently addresses them to provide needed services.
• Consults with supervisor, other clinical team staff and agency consultants.

Documentation and Reporting
• Thoroughly document all case interactions which include but are not limited to:  phone calls, emails, faxes, letters, reports and any other written correspondences within the assigned time frame. 
• Communicates as needed with the service team members about the client.
• Writes reports on client progress and participation as requested or needed by various providers.
• Makes frequent reports to supervisor on progress of client services.
• Maintains accurate, complete and timely service documentation as required by UCAN and TPSN policy and procedures.
• Enter updates and maintains accurate data in the Profile databases, including the documenting all contacts in Profile within one week of occurrence.
• Reports all critical incidents, incidents of abuse or neglect, and unusual incidents to supervisor, management, or DCFS as required by UCAN, TPSN or by mandated reporting law and document events within 24 hours of every intervention.
• Coordinate with the TPSN Resource Department regarding specific resources throughout the state.

 

• Master's Degree in Social Work or a related Human Service field.
• At least 2-3 years experience working with pregnant and parenting clients or in the Child Welfare System.
• Supervisory experience preferred.
• LCSW or LCPC preferred.
